Based on observation, interview and record review, the facility failed to prevent the worsening of facility acquired Stage II pressure ulcer for a resident and it failed to implement interventions to address the specific needs of this resident.

On March 22, 2016, this resident was in a small dining room during activities. The resident was noted to be wearing closed shoes. On the next day, the resident was noted sleeping with her head resting on the table. A certified nursing assistant (CNA) described the resident as confused and disoriented, dependent on staff with all of her activities of daily living and incontinent of bowel and bladder.

The Treatment Coordinator of Avanti Wellness & Rehab explained that the resident acquired a Stage II pressure ulcer on her right heel. It was found that the right heel wound was 100% necrotic measuring 1.7 cm x 1.5 cm. The treatment nurse identified this wound as a Stage II ulcer. The wound should have been identified as an unstageable pressure ulcer. The National Pressure Ulcer Advisory Panel identifies unstageable/unclassified pressure ulcers as a full thickness tissue loss in which actual depth of the ulcer is completely obscured by slough in the wound bed. This resident’s plan of care was not individualized based on her needs. The intervention listed included the following: continue with current wound care treatment. Keep skin dry, monitor meals, notify physician and family with any changes in pressure, relieving mattress and chair cushion.

The rating for health inspection was considered average for Avanti Wellness & Rehab. There were six total health deficiencies at this health inspection. There were also two complaint inspections that were held between October 1, 2015 and September 30, 2016.
