Alden Lakeland Rehab and Healthcare Center

Alden Lakeland Rehab and Healthcare Center has 300 certified nursing home beds. The facility participates in Medicare and Medicaid. Its ownership is a corporation which runs the nursing home as a for-profit business. The overall rating is below average. Alden Lakeland Rehab and Healthcare Center is located in Chicago’s Uptown neighborhood on the north side of the city.

The Illinois Department of Health and Human Services routinely inspects Illinois nursing homes every 18 months. In June 2015, Alden Lakeland Rehab and Healthcare Center was inspected. Based on observation and interview, the facility failed to ensure the plan of correction for the last annual survey was appropriately placed in the facility survey binder. This failure has the potential to affect all of the residents in the facility.

Another issue based on observation, interview and record review, the facility failed to provide a therapeutic diet for one of four residents reviewed for dietary concerns in a sample of 24. The findings included a resident who was given a single portion (3 oz.) or chopped beef with gravy, a half cup of wild blend rice and a half cup of vegetable blend. The facility’s dietary spreadsheet documents that this was only one portion. The facility was simply underserving its residents. The administrator and corporate partner of the facility stated that the resident’s dietary requirements does document the portions and it was an oversight by the dietary staff that the resident was underserved.

With respect to staffing, Alden Lakeland Rehab and Healthcare Center is considered average. The total number of licensed nurse staff hours per resident per day was listed as 1 hour and 25 minutes. The Illinois average is 1 hour and 34 minutes and the national average is 1 hour and 41 minutes. Alden Lakeland Rehab and Healthcare Center shows that registered nurse hour per resident per day are 1 hour and 22 minutes. This is above the Illinois and national averages.
