It is just miles to the southwest of Chicago’s city limits off of Illinois Interstate 55 or the Stevenson Expressway. Stickney is located just to the southwest of Cicero, Illinois. The Illinois Department of Health and Human Services regularly inspects Illinois nursing homes on average approximately every 18 months. One of these inspections was done of Pershing Garden Healthcare Center that was completed on February 26, 2016.

Based on observation, record review and interview, Pershing Garden Healthcare Center failed to provide psychosocial therapy to one resident in a sample of twelve residents. This particular resident was using antidepressant medication. The nursing home’s assessment dated July 21, 2015 confirmed this resident for psychosocial treatment. The resident was identified with a disease and condition that may impede abilities to interact with others and health factors that may inhibit social involvement. The care plan dated April 7, 2015 noted that this resident required psychosocial therapy related to alcohol abuse.

The resident was to begin psychosocial therapy once approved by a physician. Once approval was obtained, social services was to be set up for psychosocial therapy. The consent for this therapy was signed by the resident and a physician. However, the administrator stated that the resident did not attend the psychosocial therapy. There was no note that the resident ever attended any of the psychosocial therapy sessions that were held on Saturdays.

In another issue with Pershing Garden Healthcare Center, it was found in this inspection that the facility failed to follow its policy on kitchen sanitation. The kitchen supervisor was observed grinding pork while wearing long dangling earrings. The kitchen supervisor was also wearing a diamond wedding band while stirring the ground meat. Her hands were not gloved. Another dietary aide had long hair hanging from underneath the hairnet. Further, the dietary aide was preparing lunch for residents with a dirty apron.
